CPT 27278 (minimally invasive SI joint fusion): commercial payer coverage 2026
Which US commercial payers cover minimally invasive sacroiliac joint fusion billed as CPT 27278, and which deny it. One row per payer, with the policy number, the stated date, the device that is covered, a verbatim quote, and a link to the payer’s own policy.
Last checked 2026-10-09. Sources are each payer's own public document. Where a payer has no readable public policy, this page says "could not verify" rather than guess a stance.

27278 and 27279 split in 2026
27278 replaced Category III code 0775T on 2024-01-01 (not 2025).
The descriptor was revised effective 2026-01-01: 27278 now covers intra-articular devices that do not pierce the cortices, and 27279 covers devices that do. Several payers rewrote their code tables after that change.
Most reviewed commercial payers cover SI joint fusion only with an implant that traverses and transfixes the joint (a lateral or transiliac titanium device). They deny posterior or intra-articular allograft approaches. 27278 is the code those denials ride on.

What each payer says
The quote is the payer's own, verbatim and under 30 words, linked to the source. The classification is the payer's, not ours.
UnitedHealthcare Unproven / not medically necessary (by device)
Sacroiliac Joint Interventions, 2026T0621O. Effective 09/10/2026.
“Fusion of the Sacroiliac Joint ... is unproven and not medically necessary for all other indications and with any other devices not listed above”
UnitedHealthcare, uhcprovider.com (PDF)
Only MIS fusion with a titanium triangular implant (TTI) system is covered, using InterQual criteria. The evidence section names LinQ (ECRI, Apr 2024) and posterior allograft fusion (Hayes 2024) as having limited support.
Aetna Experimental / investigational
CPB 0016, Back Pain: Invasive Procedures. Last review 10/06/2026 (next 01/07/2027).
“Posterior non-transfixing sacroiliac joint fixations or use of additional joint implants with a transfixing device ("hybrid" fusions) are considered experimental, investigational or unproven”
Aetna, aetna.com CPB 0016
MIS fusion "involving transfixion of the sacroiliac joint (e.g., iFuse)" is medically necessary when criteria are met. 27279 is covered when criteria are met, except for hybrid fusions.
Cigna Not medically necessary
Musculoskeletal: CMM-611 Sacroiliac Joint Fusion and Stabilization, V1.0.2026. Effective 08/04/2026 (published 04/30/2026 per the file name).
“Minimally invasive or percutaneous SI joint fusion and stabilization using products/implants that do NOT traverse and transfix the SI joint ... is considered not medically necessary”
Only transfixing implants qualify. Joint distraction without the intent to fuse is also not medically necessary. Cigna policy 0303 is valid only for dates of service before 11/1/24; its PDF now 404s, so that point could not be re-read.
Elevance / Anthem Not medically necessary (exclusion)
Carelon Musculoskeletal: Sacroiliac Joint Fusion, MSK05-1125.1-RUC0126-v2. Effective 11/15/2025. Reaffirmed, last review 01/29/2026..
“Percutaneous or minimally invasive SI joint fusion procedures using a posterior approach for insertion of intraarticular allograft or devices combined with or without a transfixation device”
Elevance / Anthem, Carelon (PDF)
Listed under exclusions considered not medically necessary. The covered MIS technique is a transiliac or lateral approach with an FDA-approved structural fixation device. Posterior hybrids are also excluded.
Humana No current commercial policy
None found (Humana exited employer-group commercial medical in 2025). n/a.
“Humana members may NOT be eligible under the Plan for minimally invasive SIJ fusion via an approach other than lateral transiliac with placement of the implant across the SIJ”
Humana SC Medicaid policy HUM-SC-2235-001, not a commercial policy, Humana SC Medicaid (PDF)
For context only: Humana’s SC Medicaid policy HUM-SC-2235-001 (effective 06/17/2026) excludes this approach and names posterior and dorsal procedures "including those using only bone grafts". This is Medicaid, not commercial.
Centene / Ambetter Not supported
Clinical Policy: Sacroiliac Joint Fusion, CP.MP.126. Date of last revision 04/25.
“current evidence does not support SIJ fusion using implants other than those which are placed across the joint (transfixing) to promote fusion (e.g., allograft, nonmetallic implants)”
Centene / Ambetter, ambetterhealth.com (PDF)
27278 is in the table "Codes that do not support coverage criteria" (added 05/24). LinQ, CornerLoc and TransFasten are named as non-transfixing allograft devices placed through posterior approaches. The code table still prints the pre-2026 descriptor.
Blue Shield of California Could not verify
No SI joint fusion policy in the public medical policy list. n/a.
No public policy addresses 27278. Only "Sacroiliac Joint Injections" is listed. BSC may handle 27278 through a delegated vendor guideline that is not on the public list.
Highmark Could not verify
S-230-010, Lumbar Spine/Sacroiliac Fusion Surgery (2017 copy only). Effective 08/21/2017 (2017 copy, pre-27278).
The only public copy found is from 2017, before 27278 existed. No current version was found.
Kaiser Permanente (Washington) Unclear / internally inconsistent
Clinical Review Criteria: Sacroiliac Joint Procedures for Pain. Last revised 2/24/2025. Reviewed by MPC 06/09/2026..
“Percutaneous or Minimally Invasive sacroiliac joint fusion, using an FDA-approved implant, placed across the SI joint and intended to promote bone fusion, is considered medically necessary”
Kaiser Permanente (Washington), wa-provider.kaiserpermanente.org (PDF)
The non-Medicare code table lists 27278 as medically necessary when criteria are met, but the MIS criterion requires an implant "placed across the SI joint". The Medicare table lists 27278 under both covered and not medically necessary. Treat as internally inconsistent. Code tables use the pre-2026 descriptors.
BCBS Michigan Investigational
Sacroiliac Joint Fusion (Percutaneous or Minimally Invasive) for the Treatment of Low Back Pain. Effective 3/1/26.
“Due to lack of sufficient evidence to establish safety and/or efficacy over other treatments, percutaneous posterior intra-articular fusion of the SI joint is considered investigational”
BCBS Michigan, bcbsm.com (PDF)
27278 is the only entry under "Other codes (investigational, not medically necessary, etc.)". 27279 and 27280 are established codes. The device table lists LinQ (PainTEQ, bone allograft, HCT/P). Vendor-managed by TurningPoint. Next review 4th Qtr 2026.
Premera Blue Cross Investigational (by device)
6.01.527, Diagnosis and Treatment of Sacroiliac Joint Pain. Effective 02/01/2026. Last revised 05/01/2026..
“Fixation/fusion of the sacroiliac joint ... with any other devices other than a titanium triangular implant (i.e., iFuse, iFuse 3 D) is considered investigational”
Premera Blue Cross, premera.com (PDF)
On 05/01/26 Premera removed the code-level "investigational" label from 27278 because the 2026 descriptor covers several devices. Coverage now depends on the device, and only TTI is covered. Table 2 lists LinQ (PainTEQ, bone allograft, HCT/P).
Excellus BCBS Experimental / investigational
7.01.93, Sacroiliac Joint Fusion or Stabilization. Effective 10/15/2026 (annual review 06/18/26).
“Minimally invasive or percutaneous SIJ fusion and stabilization using products/implants that do NOT traverse and transfix the sacroiliac joint (e.g., allograft wedge ...) are considered investigational”
Excellus BCBS, excellusbcbs.com (PDF)
The code table shows "27278 (E/I)". Implants must "traverse and transfix". The wording matches Cigna/EviCore CMM-611. The rationale names LinQ (Calodney 2024 SECURE study).
What changed recently
- 2026-01-01 descriptor revision: Premera (05/01/26) removed the code-level "investigational" label from 27278 because the code now covers several devices. Coverage still depends on the device, and only TTI is covered. Carelon and Excellus updated their code tables with no change in stance.
- Cigna/EviCore CMM-611 V1.0.2026 (effective 08/04/2026) keeps non-transfixing and allograft-wedge implants "not medically necessary". UHC 2026T0621O (09/10/2026) changed only documentation and InterQual references; its device stance (TTI only) did not change.
- Carelon/Anthem reaffirmed the posterior-allograft exclusion on 01/29/2026. The 11/15/2025 revision restated that MIS fusion means a transiliac or lateral approach with an FDA-approved fixation device.
Medicare denies the non-transfixing approach too
Context only, not a commercial policy. Palmetto GBA LCD L39797, Minimally Invasive Arthrodesis of the SIJ, effective 04/17/2025, denies the minimally invasive approach performed without a transfixation device.
“MI Arthrodesis of the SIJ WITHOUT placement of a transfixation device is NOT considered medically reasonable and necessary.”
Palmetto GBA, LCD L39797
CGS and Noridian published parallel LCDs at the same time (not opened in the snapshot). No NCD found.
Why these deny, and how the appeal works
No patient data. No BAA. No charge.
Most commercial 27278 denials come down to one thing: the payer covers SI joint fusion only with a transfixing implant, and reads the device used as posterior or intra-articular. An appeal has to answer that specific policy with the policy’s own words, the operative note and the device. Send us 90 days of denials, 4 columns, and you get back which 27278 lines are worth working and what they are worth. The first review is free.
